This is The Physio Nook’s (“The Physio Nook”) privacy policy. This document explains The Physio Nook’s policies for the collection, use and disclosure of personal information. The Physio Nook abides by applicable privacy legislation. The Physio Nook is a physiotherapist office. The Physio Nook uses a third party telemedicine solution based in the United States to provide remote services, which does not itself collect or retain personal health information.

The Physio Nook collects information primarily by information actively provided by its patients, or from other parties as authorized by patients. The types of personal information we collect includes health information relevant to our services, and payment information.
HOW WE USE THIS INFORMATION
This information is used to provide our services to our patients. The Physio Nook may use aggregate or anonymous information which will not be linked to identified individuals for various other uses for itself and third parties.
WHO WE SHARE THIS INFORMATION WITH
The Physio Nook does not share personal information with any third parties except as disclosed in this policy. The Physio Nook may provide personal information to its consultants, subcontractors and professional advisers (all which shall be bound by privacy obligations) to the extent necessary to assist The Physio Nook’s patient services.

Personal information is stored in a combination of paper and electronic files. They are protected by security measures appropriate to the nature of the information.
ACCESSING INFORMATION
Individuals may review their personal information contained in The Physio Nook’s files by contacting the The Physio Nook’s privacy officer. If an individual believes that any of their personal information is inaccurate, we will make appropriate corrections.
GENERAL
The Physio Nook may amend this policy from time to time by posting an updated version on our website. If such amendments affect how The Physio Nook uses or discloses personal information already held by The Physio Nook in a material way, The Physio Nook will obtain consent.
